Lone Sage Elixir to Thin the Pox

Dokseong-dan

한글 독성단  ·  Pinyin Du Sheng Dan

獨聖丹

The name is close to an entry already registered: `dokseong-san` 獨聖散, the melon-pedicel emetic of the Uigam and Dansim for wind, epilepsy and welling phlegm. That is a different prescription of a different dosage form, and this leaf takes the id its own 丹 tail gives. 搗 is normalized to 擣 and 𤍠 to 熱. The sentence 小兒痘疹, 服此則少, 或全然只燒蒸三兩日不出者 reads awkwardly and is punctuated here as it stands without emendation; 全然 is taken with 不出. The formula is cited again in the next section, 1.61 痘瘡預防法, where 獨聖丹ㆍ兔紅丸 are recommended together — that passage is prose and belongs to another worker. No entry of this name existed in the database.

Translation

Take three inches of luffa from the calyx end, skin and seeds together, put it into a sand jar luted shut, burn it over a mulberry-wood fire until it is charred but its nature is kept, and powder it; mix in an equal measure of granulated sugar, pound it into cakes, and from time to time give them to the child until they are all eaten, which is best. When a child has the pox, taking this makes it scanty, or else the child is merely feverish for two or three days altogether and nothing erupts. Or whenever the heat rises, give it at once to eat, and the pox that comes out will certainly be few. 《醫鑑》 ○Whenever the pox is first erupting, or before it erupts, taking this drug makes many become few, few become none, and a severe case become mild. It is also said that adding a little cinnabar is better still. — Jeongjeon 正傳 (Donguibogam · Soa)

Clinical commentary 臨床

One drug, which is what 獨聖 means: three inches of luffa from the stem end, skin and seed left in, sealed in a luted jar and burnt over mulberry wood until it is charcoal that still holds its form, then ground and mixed with its own weight of sugar and pounded into cakes. Sugared charcoal, given to a child a piece at a time until it is gone. The Jeongjeon's clause extends it from prophylaxis to the first days of the eruption, and sets out the same graded claim the rest of the section makes: many pocks become few, few become none, a severe case becomes mild. Carbonised plant matter does nothing to variola. The claim about density is again a claim about the variable that genuinely predicted death — a discrete eruption carried roughly a tenth the mortality of a confluent one — and the formula had no purchase on it. The second half of the Uigam's clause, that a child given this is merely feverish for two or three days and nothing comes out, describes a child who did not have smallpox. Taken as written in the first clause this is the least harmful preparation in the section: charcoal and sugar, which would do a child neither good nor harm. **The Jeongjeon's final sentence is where it becomes dangerous, and it must not be followed: 入朱砂少許, add a little cinnabar.** Cinnabar is mercuric sulfide, the patient here is a well child, and 少許 is not a quantity — **the leaf gives no dose by weight for the child here**, so a cake eaten over days delivers an unmeasured mercury dose to a developing nervous system that does not repair the injury. That one clause, appended as an improvement, is the chapter's reflex: where a method appears to be doing no harm, cinnabar is added. The full statement of what cinnabar is and does to a child is written out on `jinsa-go-soa`. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
