Donguibogam 東醫寶鑑

治上焦鬱火, 痰涎壅盛, 胸膈不利, 咽喉煩燥, 噎塞如有所硬, 吐不出, 嚥不下. 大黃(酒拌九蒸九曬) 二兩半, 白朮(炒)ㆍ枳實(麩炒)ㆍ陳皮(鹽水炒) 各二兩, 黃芩(酒炒)ㆍ黃連(酒炒)ㆍ梔子(炒)ㆍ南星ㆍ半夏(二味, 以白礬ㆍ皂角ㆍ生薑 各一兩, 浸七日) 各一兩半, 貝母(炒) 一兩三錢, 連翹ㆍ天花粉ㆍ白茯苓ㆍ神麴(炒)ㆍ白芥子(炒) 各一兩, 玄明粉 七錢, 靑礞石(以焰硝一兩同煆如金色)ㆍ靑黛ㆍ甘草 各五錢, 沈香 二錢. 右爲末, 竹瀝和丸梧子大, 茶淸下六七十丸. 《醫鑑》

Translation

Treats depressed fire in the upper burner with phlegm-drool congesting and abundant, chest and diaphragm not free, throat vexed and dry, a choking obstruction as though something hard were lodged there that will neither be spat out nor swallowed down. Rhubarb (wine-mixed, steamed and sun-dried nine times over) 2.5 nyang; white atractylodes (stir-fried), bitter orange (bran-fried) and tangerine peel (salt-water fried) 2 nyang each; scutellaria (wine-fried), coptis (wine-fried), gardenia (stir-fried), arisaema and pinellia — the last two steeped seven days with alum, gleditsia pod and fresh ginger, 1 nyang each — 1.5 nyang each; fritillaria (stir-fried) 1 nyang 3 don; forsythia, trichosanthes root, white poria, medicated leaven (stir-fried) and white mustard seed (stir-fried) 1 nyang each; refined mirabilite 7 don; chlorite schist, calcined to a gold colour with 1 nyang of niter, indigo and licorice 5 don each; aloeswood 2 don. Powder them, make pills with bamboo sap the size of a firmiana seed, and take sixty or seventy down with clear tea. — Gogeum Uigam 醫鑑 (Donguibogam · Damum)

Clinical commentary 臨床

Twenty medicinals, and nearly every one of them processed — nine steamings for the rhubarb, wine for the two bitter colds, salt water for the tangerine peel, a seven-day soak for arisaema and pinellia, calcination with niter for the chlorite schist. The processing is where the formula's judgement lives. Rhubarb steamed nine times has lost most of its purgative force and keeps its heat-draining action, which is what lets it lead a formula meant for the throat rather than the bowel. The indication is globus, described here with the precision the Bogam reserves for symptoms patients report in their own words: something hard lodged there that will not come up and will not go down.
