Needle Substitute Pill, Second Formula

Chechim-hwan

한글 체침환 (둘째)  ·  Pinyin Ti Zhen Wan

替鍼丸

The chapter prints two formulas under this name, one after the other. This is the second, the Samin leaf; the first, the Jeongyo leaf with the lime lye, is registered as chechim-hwan. 治同上 points back to that leaf for the indications. 䃃砂 is the form this corpus prints for the sal ammoniac, and 粟米大, a millet grain, is a size and not a drug.

Donguibogam 東醫寶鑑

治同上. 白丁香 二十粒, 䃃砂ㆍ沒藥ㆍ陳倉米 各一字. 右硏勻, 以米飯和丸粟米大, 貼瘡上, 卽潰膿出. 《三因》

Translation

Treats the same as above. Sparrow droppings, 20 grains; sal ammoniac, myrrh and aged granary rice, one ja each. Grind the above even, bind with cooked rice into pills the size of a millet grain, and stick one on the sore; it bursts at once and the pus comes out. — Samin 三因 (Donguibogam · Ongjeo)

Clinical commentary 臨床

The same idea as the leaf above it and a far shorter one: no lye, no overnight soaking, four powders bound with cooked rice into a millet-sized pill and stuck on the sore. Sal ammoniac is the caustic that does the work here, with twenty grains of sparrow droppings — an alkaline, irritant material the tradition used precisely for eating through a hardened sore head — and myrrh for the pain. Aged granary rice is the bulk. Two formulas of one name, printed together, is how the Bogam records a method rather than a recipe: what is being passed on is the practice of opening an abscess chemically instead of with a lancet, and either compound will do it. The principle underneath — pus has to come out — is still the principle. **Sal ammoniac and sparrow droppings applied to break open skin make this a caustic preparation, and it must not be reproduced.** Abscess drainage is a sterile procedure, and an abscess opened with a chemical burn leaves a wound that is larger and dirtier than the one a blade makes. — Eui Young (Justin) Chung, L.Ac., Dipl.
