Clinical commentary 臨床

The formula is named for 石菖蒲, acorus — the rhizome of *Acorus gramineus*, the pungent aromatic the tradition uses to "open the orifices", and the standing drug for a blocked tongue, deafness and dulled senses. It is the chief here for exactly that reason: the section reads failure to speak as the root of the tongue not being patent (舌本不通) because the Heart spirit is short, and acorus is the drug chosen to force the passage open. **It also contains cinnabar, 朱砂, which is mercuric sulfide — a mercury compound, 1 don of it in the batch, in pills given ten and twenty at a time to a five-year-old. The text gives no dose by weight here. This must not be prepared.** The full statement on mercury in this chapter is written at 辰砂膏 (`jinsa-go-soa`); the short form is that there is no safe dose at any age, that the developing nervous system is the organ at risk, and that the injury does not reverse. In a child who is not speaking, a drug that damages the developing brain is the worst possible choice, and that is the whole of what needs saying about this prescription. **A child who is not talking needs a cause found, and the one that is both common and fixable is deafness.** A child who cannot hear does not learn to speak, and hearing loss severe enough to prevent speech is invisible to a parent — the baby is settled, responds to being picked up, turns when a room brightens. That is why universal newborn hearing screening exists, and why it is done in the first days of life rather than waited for: a hearing aid or a cochlear implant fitted in infancy gives spoken language, and the same device fitted at five gives far less, because the auditory cortex has a window. The other fixable causes behind delayed speech are glue ear with a conductive loss, and a severely impoverished language environment. Behind the rest sit autism, global developmental delay and specific language disorder, all of which respond to early intervention — speech and language therapy, and in autism early behavioural support — and all of which do better the earlier they start. The modern red flags are plain: **no words by 18 months, not walking by 18 months, and above all the loss of a skill the child already had**, which is not delay at all and needs urgent assessment. Five or six years old, as this leaf describes, is very late to be starting. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
