Donguibogam 東醫寶鑑

治老瘧腹中結癥瘕, 名曰瘧母. 鼈甲 二錢, 白朮ㆍ黃芩ㆍ草果ㆍ檳榔ㆍ川芎ㆍ陳皮ㆍ厚朴ㆍ白芍藥 各一錢, 甘草 五分. 右剉, 作一貼, 入薑三棗二梅一, 水煎服. 《綱目》

Translation

Treats old malaria with a concretion and conglomeration bound in the abdomen — this is called the malaria mother. Turtle carapace 2 don; white atractylodes, scutellaria, tsaoko fruit, areca seed, cnidium, aged tangerine peel, magnolia bark and white peony, 1 don each; licorice 5 fun. Cut up, make one packet, add 3 slices of ginger, 2 jujubes and 1 mume plum, decoct in water and take. — Gangmok 綱目 (Donguibogam · Haehak)

Clinical commentary 臨床

The decoction counterpart to Byeolgap-hwan, and a much gentler piece of work. Turtle carapace still leads, but at two don rather than a full nyang, and instead of the pill's sparganium, zedoary, peach kernel and safflower there is only cnidium and white peony moving blood. What fills the space is harmonizing: white atractylodes, aged tangerine peel, magnolia bark, licorice, ginger and jujube. That is a deliberate trade. A patient who can be given a pill of sparganium and zedoary for weeks is a different patient from one who needs the centre supported while the mass is worked on, and this formula is written for the second. Scutellaria is the one cold drug in it, left in because the heat of the paroxysms has not finished; areca seed and tsaoko fruit carry the aromatic, damp-drying line the chapter uses throughout. The mume plum at one fruit per packet does here what it does in Yangwi-dan — astringes against a formula that is otherwise all dispersal. A palpable abdominal mass in someone with years of relapsing fever is most often a malarial spleen, and it needs imaging and a parasitological diagnosis; the spleen in that state can rupture. — Eui Young (Justin) Chung, L.Ac., Dipl.
