Donguibogam 東醫寶鑑

凡飮食不進, 由房勞過度, 眞火衰弱, 不能熏蒸脾土, 以致中州不運, 胸膈痞塞, 飮食不消. 古人云, 補腎不如補脾, 予謂補脾不若補腎. 若丹田眞火上蒸脾土, 脾土溫和, 則膈開能食, 此藥主之. 鹿茸ㆍ巴戟ㆍ鍾乳粉 各一兩, 葫蘆巴ㆍ香附子ㆍ陽起石ㆍ川烏ㆍ肉蓯蓉ㆍ兎絲子ㆍ沈香ㆍ肉豆蔲ㆍ五味子 各五錢. 右爲末. 用羊腰子兩對, 以葱椒酒煮爛, 和酒爲糊擣丸梧子大, 每百丸, 空心, 米飮ㆍ鹽湯任下. 《本事》

Translation

Whenever food and drink are not taken in, it comes of sexual taxation carried to excess, the true fire declining and weakening so that it cannot steam the spleen earth; hence the central province does not move, the chest and diaphragm are blocked, and food is not digested. The ancients said that supplementing the kidney is not as good as supplementing the spleen; I say that supplementing the spleen is not as good as supplementing the kidney. If the true fire of the cinnabar field steams the spleen earth above and the spleen earth is warm and harmonious, then the diaphragm opens and food can be eaten. This medicine governs it. Deer antler velvet, morinda and stalactite powder 1 nyang each; fenugreek, cyperus, actinolite, aconite root, cistanche, cuscuta, aloeswood, nutmeg and schisandra 5 don each. Powder them. Take two pairs of sheep's kidneys, boil them soft in wine with scallion and zanthoxylum, and pound them with the wine into a paste; form pills the size of a firmiana seed and take a hundred at a time on an empty stomach, down with rice broth or salt decoction as preferred. — Bonsa 本事 (Donguibogam · Naesang)

Clinical commentary 臨床

The passage argues with received opinion and says so: the ancients held that supplementing the kidney is not as good as supplementing the spleen, and the author reverses it. The image offered is a cooking fire — the true fire of the cinnabar field steams the spleen earth from below, and when it fails the food sits undigested however much the spleen is tonified. This is the chapter's answer to anorexia arising from depletion rather than from obstruction, and the whole formula is warm and heavy: antler velvet, actinolite, stalactite, aconite root. Sheep's kidneys boiled in wine make the binder, on the same organ-treats-organ reasoning the Oedema chapter uses at 2.5.17. **Contains aconite root (川烏), which is toxic.
