Clinical commentary 臨床

Not the Samin 補脾湯. That one is barley sprout and licorice at 1 nyang 5 don over ginseng, poria, tsaoko, dry ginger, magnolia bark, tangerine peel and white atractylodes — Ijung-tang and Pyeongwi-san together, for an adult spleen vacuous and cold. This one is the Uigam's, and it has fourteen drugs: 四君子湯 and most of 四物湯 laid over each other, with pinellia and tangerine peel to dry phlegm, nutmeg and medicated leaven for the stool and the appetite, pueraria to raise, and a small stir-fried coptis against the heat. Only ginseng, white atractylodes, white poria, licorice and tangerine peel are common to the two, and no weight agrees. They share a name and nothing else, so this leaf takes the chapter tail. 稍稍服 — given a little at a time, which is the one instruction in the leaf that fits the patient. A convulsing, vomiting infant cannot take a decoction in one draught. As a nourishing decoction for a wasted child this is about as sensible as the chapter gets, and it is still beside the point: what the child has lost is water and salt, and the reading is set out at sinhyo-san-soa. **Contains pinellia, which is a severe mucosal irritant unless properly processed, and the patient is a child. The text gives no dose by weight anywhere in this chapter.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
