Donguibogam 東醫寶鑑

專治下疳瘡. 柴胡 一錢半, 黃栢ㆍ赤芍藥ㆍ赤茯苓ㆍ木通ㆍ草龍膽 各九分, 連翹ㆍ荊芥ㆍ黃連ㆍ蒼朮ㆍ知母 各七分, 防風ㆍ獨活 各六分, 甘草 三分. 右剉, 作一貼, 燈心一團, 水煎服. 《回春》

Translation

Treats the chancre sore specifically. Bupleurum 1 don 5 pun, phellodendron, red peony, red poria, akebia and gentian 9 pun each, forsythia, schizonepeta, coptis, atractylodes lancea and anemarrhena 7 pun each, saposhnikovia and pubescent angelica 6 pun each, liquorice 3 pun. Cut them up, make one packet, add one bundle of rush pith, decoct in water and drink. — Manbyeong Hoechun 回春 (Donguibogam · Jechang)

Clinical commentary 臨床

Fourteen small doses with bupleurum at double the rest, and the structure is liver-channel: bupleurum and gentian lead into the liver, akebia and red poria drain damp heat downward through the urine, phellodendron, coptis and anemarrhena cool the lower burner, forsythia and schizonepeta disperse the toxin, and saposhnikovia and pubescent angelica open the surface. The genitals belong to the liver channel in this reasoning, so a sore there is treated as liver-channel damp heat — which is the same reasoning behind 龍膽瀉肝湯, the formula the section recommends for 陰蝕瘡 when the damp heat is severe. The chapter pairs this with Yanghyeolhaedok-hwan as the two general prescriptions for the condition — 通用消疳敗毒散凉血觧毒丸 — the decoction and the pill. 專治, treats specifically, marks it as the named remedy rather than a borrowed one. 下疳 is the chancre, and the chapter states that an 陰蝕瘡 which does not heal becomes 楊梅瘡, syphilis. That is the correct natural history seen from outside: the primary ulcer heals by itself and the disease goes on. A painless genital ulcer must be treated as primary syphilis until serology says otherwise, with testing for HIV and other sexually transmitted infections and partner notification; the cure is benzathine penicillin.
