Perilla and Pueraria Decoction

Sogal-tang

한글 소갈탕  ·  Pinyin Su Ge Tang

蘇葛湯

The transcription writes 蘓 for 蘇 at the leaf head and in 紫蘓葉; 蘓 is on the project's variant-glyph list and the corpus prints 蘇 in 279 places against twenty, so the name is registered 蘇葛湯 and the id reads from it. 鍳 is printed 鑑. The leaf gives no 治 clause; the indication stands in the section prose of 1.89, 麻疹以升麻葛根湯方見寒門加葱白紫蘇葉乃麻疹初起之神方也, 或用蘓葛湯亦佳. That sentence is the formula's own derivation — it is the Ascending Pueraria Decoction route with scallion white and perilla leaf, offered as an alternative — and 升麻葛根湯 is registered seungmagalgeun-tang and carried in `related`. No formula of this name stood in the database.

Clinical commentary 臨床

Six drugs with scallion white and ginger added, and it is transparently what the section prose says it is: the pueraria-and-scallion approach to an early measly rash, written out as its own formula. Perilla leaf and pueraria root lead, with white peony, liquorice, tangerine peel and amomum behind them. 1.89 附癍疹 is not smallpox, and it is not one disease. The section sorts rashes by what the lesions look like — 癍, coloured patches with no raised grain; 疹, small raised grains; 麻, the smallest of all, flat-topped and soft so that the finger does not catch — and under 麻 it gives a description that is largely measles: red patches coming out on the fifth or sixth day of fever, spreading over the whole body with no clear skin left, three days of fever before the eruption and three of swelling, the eyes swollen shut in a severe case, and death where the rash will not come out and the child is breathless. The breathlessness is measles pneumonia, which is how measles chiefly kills. But the same passage says the rash goes and comes and goes again over about a day, 出沒一周時許, and the measles rash does not do that; so this 麻 is not measles alone, and 癍 described as mosquito-bite marks could be petechiae, a drug eruption, scarlet fever or any of several things. The honest reading is that this section covers a family of febrile rashes of which measles is the largest member, and that it cannot be mapped onto one modern diagnosis. **Measles, unlike smallpox, still has patients.** It kills on the order of a hundred thousand children a year, mostly under five, mostly through pneumonia and through the diarrhoea and malnutrition that follow; it causes encephalitis in roughly one case in a thousand and subacute sclerosing panencephalitis years later in a few; and it suppresses immunity for months afterwards. Two things matter more than anything in this formula. **A safe and highly effective vaccine exists, and measles returns wherever coverage falls.** And **vitamin A reduces measles mortality** — it is cheap, well evidenced, and recommended by the WHO for every child with measles. A warm diaphoretic decoction may make a feverish child more comfortable; it does not alter the course of measles, and bringing the rash out faster was never the thing that decided whether the child lived. — Eui Young (Justin) Chung, L.Ac., Dipl.
