Donguibogam 東醫寶鑑

治膏粱之人, 濕熱傷腎, 脚膝痿弱無力. 龜板(酥灸) 四兩, 黃柏ㆍ知母(幷鹽水炒) 各二兩, 枸杞子ㆍ五味子ㆍ鎖陽 各一兩, 乾薑 五錢. 右爲末, 酒糊和丸梧子大, 鹽湯下七十丸. 《綱目》

Translation

Treats damp heat damaging the Kidney in one whose fare is rich and fatty, with legs and knees wilting, weak, and strengthless. Butter-roasted turtle plastron 4 nyang; phellodendron and anemarrhena, both salt-water-fried, 2 nyang each; lycium fruit, schisandra, and cynomorium, 1 nyang each; dried ginger 5 don. Powder the above and bind with a wine paste into pills the size of parasol-tree seeds; take 70 pills with salted broth. — Gangmok 綱目 (Donguibogam · Jok)

Clinical commentary 臨床

Four ounces of turtle plastron leading, and the indication names the patient precisely: 膏粱之人 — one whose daily fare is rich and fatty — in whom damp heat has damaged the Kidney. That is the classical description of a metabolic patient, and the formula treats it by enriching rather than by draining alone.
