Bupleurum Four Substances Decoction

Sihosamul-tang

한글 시호사물탕  ·  Pinyin Chai Hu Si Wu Tang

柴胡四物湯

The Bogam carries two different formulas under the name 柴胡四物湯: this one, from the Haehak chapter, for warm malaria of the three yin channels and for malaria that breaks out at night, while the Buin chapter's Bomyeongjip formula of the same name treats postpartum fever with heat entering the chamber of Blood. The leaf itself prints no source marker; the chapter's own 保命 passage above it names the formula — 或小柴胡合四物湯名曰柴胡四物湯 — and the attribution follows that passage.

Translation

Treats warm malaria of the three yin channels, and the kind that breaks out at night. Bupleurum and dried raw rehmannia, 2 don each; ginseng, pinellia, scutellaria, licorice, cnidium, Korean angelica and red peony, 1 don each. Cut the above, make one packet, add three slices of ginger and two jujubes, and decoct in water and take. (Donguibogam · Haehak)

Clinical commentary 臨床

The chapter says what it is in so many words: Sosiho-tang joined to Samul-tang. The join is not quite even — bupleurum and rehmannia stand together at two don as the two chiefs, and rehmannia is the raw dried kind rather than the prepared one of Samul-tang, so the blood side cools as well as fills. The indication is the chapter's reason for reaching for blood medicinals at all. Dansim's rule runs 老瘧係風暑之邪入在陰分宜用血藥引出陽分而散 — an old malaria is wind and summerheat gone into the yin division, and blood medicinals are what lead it back out to the yang to be scattered. A fit that breaks out at night is the sign that the evil has gone that deep, which is why the leaf names night onset alongside the three yin channels. **Contains pinellia, which is toxic raw and must be properly processed before use.** — Eui Young (Justin) Chung, L.Ac., Dipl.
