Wide-Awake Powder

Seongseong-san

한글 성성산  ·  Pinyin Xing Xing San

惺惺散

The transcription writes 薑二 without the measure word; elsewhere in this chapter the same instruction is written 薑二片, two slices, and it is read that way here. 各二分半 is read as 1 g on the chapter's conversion. The section prose of 1.44 感冒風寒 names this leaf as the general formula for a wind-cold cold, alongside 人參羌活散 and 參蘇飮.

Clinical commentary 臨床

Four-fifths of 四君子湯 and 白芍藥 at its centre, with 桔梗 for the cough, 瓜蔞根 for the thirst, and the smallest possible amounts of 細辛 and 薄荷 to open the exterior. It is a gentle formula for a child with an ordinary cold, and the name — wide-awake — describes what it is meant to restore rather than what it contains. 1.44 感冒風寒 is the common cold, and the useful thing to say about it is short. Nothing cures a cold. It is caused by any of a few hundred respiratory viruses, it lasts about a week, and the cough can outlast the rest by two or three. No decoction, antibiotic, decongestant or cough syrup shortens it, and over-the-counter cough and cold preparations are not recommended under the age of six because they do not work and have harmed children. Fluids, rest and time are the treatment. What matters is knowing when it is no longer a cold, and the sign is the breathing. Pneumonia is the complication that kills children, and it announces itself in the chest rather than the thermometer: breathing that is fast and does not settle when the fever comes down, the skin drawing in between or below the ribs with each breath, nasal flaring, a grunt at the end of expiration, a child too breathless to feed, blue or grey colour. Any of those is a same-day assessment. So is a child who cannot be roused or consoled, who is not drinking, or who has stopped passing urine. **Contains asarum, which is of the Aristolochiaceae and carries aristolochic acid and safrole; aristolochic acid is nephrotoxic and carcinogenic and there is no threshold below which it is known to be safe. The dose here is small, but the text gives no dose by weight anywhere and the patient is a child.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
