善應膏

This is the leaf 癰疽門 points at. Its entry there carries the indications and the pill instruction 作丸如上法服 but no composition, referring it away with the interlinear note 方見雜方; this leaf supplies the composition and closes that composition-less cross-reference. 瀝靑, pitch, has no identifier in the herb list — 松脂, pine resin, is a different drug — and is left out of the ingredients. 葱白 is added later with a second portion of hair and is given no weight. The 癰疽門 entry carries no source label; this leaf gives 《得效》, so the source is entered from it. 黄 is printed 黃, 𢚱 as 葱 and 聼 as 聽 by the usual normalisations; a collation note 𢚱 : 葱(First Edition) stands at the foot of the leaf and is a pure glyph variant already covered.

Translation

Red lead 8 nyang; styrax, frankincense and myrrh (all ground separately), angelica sinensis, angelica dahurica, apricot kernel, rhubarb, wild aconite, Sichuan aconite, red peony, areca, dried fresh rehmannia, chuanxiong, pitch (ground separately) and tangled hair, 1 nyang each. Steep the drugs in 1 geun of sesame oil three nights, render over a slow fire until black, then put in scallion white and tangled hair and boil a little while; strain off the dregs, set it back on the slow fire and boil it down, and add the red lead, stirring without letting the hand rest, with a willow paddle, until a drop in water forms a bead that does not scatter. Then add the frankincense, myrrh and styrax, stir thoroughly even, put it in a vessel, sink it in water three days, and take it out ready for use. It treats malignant sores and toxic swellings of every kind, and injuries of every kind. — Deukhyo 得效 (Donguibogam · Japbang)

Clinical commentary 臨床

The last of the six great plasters and in substance a doublet of 雲母膏 — the same double use, outside and in, and 如上法 sends the reader back to that leaf for how the pills are made and swallowed. Its own contribution is the injury list: 閃肭 for a wrenched joint, 杖瘡 for the wounds of a beating, 疥癬 for the itching skin diseases. A household plaster, kept for whatever broke the skin. 善應, ready response, is a plaster-maker's name and says nothing about what is in it. The recovered composition explains the injury list. 當歸, 川芎, 赤芍藥, 大黃, 乳香 and 沒藥 are the blood-moving core of the trauma formulas, and the two aconites are there for pain. That makes this the one plaster of the group assembled for bruises and wounds rather than for abscesses, and the 諸傷 at the end of its own indication line — injuries of every kind — is the leaf agreeing. The sinking in water for three days is a curing step, as the burying in earth is for 萬應膏. **Contains wild aconite (草烏) and Sichuan aconite (川烏), which are toxic, and red lead (黃丹), which is lead tetroxide.** Both aconites are present at full weight. The route that matters for an ointment is absorption through broken skin: aconitine crosses abraded skin readily and is cardiotoxic, and this plaster is meant for exactly the broken surfaces — beatings, bites, metal wounds — where it would. Lead is a cumulative poison with no safe dose, and this leaf also directs the mass to be swallowed as pills. **This must not be prepared.** The internal clause is printed because the leaf prints it, not because it should be followed. — Eui Young (Justin) Chung, L.Ac., Dipl.
