Clinical commentary 臨床

子母俱服 — mother and child both take it. The Jeongjeon is dosing the nursing mother on the assumption that what she takes reaches the baby through the milk, which is how the chapter handles an infant too small to swallow a decoction. Drugs do pass into breast milk, so the mechanism is not imaginary; the quantities that cross are small and unpredictable, which makes it a route for harm as readily as for benefit, and the scorpion in the formula is the part that should give pause. The body of it is 六君子湯 — ginseng, white atractylodes, white poria, licorice, pinellia, tangerine peel — with gastrodia for the wind, a little asarum and mint, and one scorpion. Twenty-nine grams of crude drug, two thirds of it ginseng and atractylodes, so as a decoction for a wasted child it is at least a food. **Contains scorpion, a neurotoxic animal venom whose dose in dried material cannot be measured, and pinellia, a severe mucosal irritant unless properly processed. The patients are a child and a nursing mother, and the text gives no dose by weight anywhere.** The reading for this section is at sinhyo-san-soa. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
