Donguibogam 東醫寶鑑

治風痰, 痞積, 眩暈, 喉痺, 癱瘓不語, 腹中痞塊等證神效. 小胃丹一劑方見下, 加南星ㆍ半夏(幷用白礬ㆍ皂角ㆍ薑汁水煮十五次) 各二兩半, 蒼朮(用米泔ㆍ白礬ㆍ皂角水浸一宿炒) 二兩, 桃仁ㆍ杏仁(幷以白礬ㆍ皂角水泡去尖)ㆍ紅花(酒蒸)ㆍ陳皮ㆍ枳實(幷用白礬水泡半日炒)ㆍ白朮ㆍ白芥子(炒) 各一兩. 右爲末, 薑汁, 竹瀝煮神麴爲糊和丸菉豆大, 每服二三十丸, 薑湯下. 《入門》

Translation

Treats wind phlegm, glomus accumulation, dizziness, throat impediment, hemiplegia with loss of speech, and glomus lumps in the abdomen, with marked effect. Take one batch of Minor Stomach Elixir, whose formula appears below, and add: arisaema and pinellia, both boiled fifteen times in water with alum, gleditsia pod and ginger juice, 2.5 nyang each; atractylodes lancea, steeped overnight in rice-washing water with alum and gleditsia pod then stir-fried, 2 nyang; peach kernel and apricot kernel, both soaked in alum and gleditsia pod water with the tips removed, safflower steamed in wine, tangerine peel, bitter orange soaked half a day in alum water then stir-fried, white atractylodes, and white mustard seed stir-fried, 1 nyang each. Powder them, boil medicated leaven in ginger juice and bamboo sap to make a paste, form pills the size of a mung bean, and take twenty or thirty at a dose down with ginger decoction. — Uihak Immun 入門 (Donguibogam · Damum)

Clinical commentary 臨床

Fifteen boilings for the arisaema and pinellia is the most laborious processing anywhere in the chapter, and every single medicinal here except the atractylodes pair passes through alum or gleditsia pod water first. The section's premise explains why: lumps that sit between skin and membrane are damp phlegm that flowed, stopped and hardened into nodules, and a formula meant to shift them has to be strong enough to be dangerous unprocessed. Peach kernel and safflower are the blood movers — a phlegm nodule that has been in place long enough acquires a blood-stasis component, and the chapter says elsewhere that phlegm may travel with static blood. **Any new or growing lump requires medical evaluation before it is treated as phlegm.** The section's own description — hard as stone, no pus on incision, sometimes thin blood or clear fluid, movable and egg-sized under the skin — covers presentations that today would need imaging and biopsy to distinguish. — Eui Young (Justin) Chung, L.Ac., Dipl.
