Donguibogam 東醫寶鑑

治寒濕相合, 腦痛, 脊骨, 胛眼痛, 膝臏痛. 蒼朮四兩, 水二椀煎至一椀去滓, 入羌活ㆍ升麻ㆍ澤瀉ㆍ柴胡ㆍ藁本ㆍ白朮各五分, 黃柏三分, 紅花少許. 右剉, 入蒼朮湯內再煎至半, 去滓服. 《丹心》

Translation

Treats cold and damp combined, with pain of the head, the spine, the scapular hollow and the knee and patella. Black atractylodes 4 nyang, decocted in 2 bowls of water down to 1 and strained. Then add notopterygium, cimicifuga, alisma, bupleurum, ligusticum root and white atractylodes 0.5 don each, phellodendron 0.3 don, and a small amount of safflower. Cut these, put them into the atractylodes decoction, decoct again down to half, strain and take. — Dansim 丹心 (Donguibogam · Bae)

Clinical commentary 臨床

The formula is named for its method. Black atractylodes at 4 nyang is decocted alone and strained, and everything else is then cooked in that liquid — so the atractylodes becomes the solvent rather than a co-ingredient, and the disproportion between 150 grams of it and fractions of a gram of the rest is deliberate. The design says the damp must be dried before anything else can act. Note the very small safflower: a trace of Blood-moving in an otherwise drying formula, marking pain that has become fixed. 胛眼 — the scapular hollow, the depression medial to the scapular spine — is a precise localization and corresponds to where rhomboid and levator complaints are felt. — Eui Young (Justin) Chung, L.Ac., Dipl.
