Translation

Rhubarb, coptis and scutellaria, 2 don each, cut up; blast-fried aconite, 2 don, decocted separately to a liquor. Take a cup of water brought to a hundred boils, steep the three drugs in it, after a good while strain them off, add the aconite liquor, and take warm in two divided doses. — Junggyeong 仲景 (Donguibogam · Han, lower)

Clinical commentary 臨床

One of the most deliberate preparations in Zhongjing, and the method is the formula. The three cold drugs are steeped in just-boiled water and strained — never decocted — so that only their light, surface-clearing portion comes out; the aconite is decocted hard and separately, so its warming substance comes out fully. Then the two are combined. The pattern needs opposite things at once: heat bound in the epigastrium above, yang failing below, evidenced by sweating and chill in a patient who also has stuffiness. Decoct them together and you lose both effects. Aconite is toxic and requires a properly processed product and a trained prescriber. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
